If you have a pin shear off. (you will know it immediately). Do not drive the truck. Shut it off as soon as possible. Have it towed. Continuing to drive the truck wil allow the balancer to slip on the crank and it can play hell on the nose of the crank and the inside of the balancer.
Pour the CLR into the bucket and add the steel lines. let them sit like 30 to 45 minutes. The rust is gone and lines look new. To spin 6+k rpm reliably in those it should e balanced to less than 1 gram. Also told the diesel cranks are way worse than the gasoline motors when it comes to being balanced... Mack: 2004 lb7.

If you think it might be a rod bearing, pull the filter cut it open, unroll the filter media and take a look for metal, you can also order an oil sample kit, take the sample send it in and it will also tell the tale, if its a bearing. Icepick all the injector connectors, reinsulate the the FICM harness at the alternator bracket and do the DIY injector cleaning. After that, start by replacing the injector that is the worst, then check balance rates again. Ideally you would first run a compression test on any cylinders with high positive balance rates. Once you remove that hub then you can see the cam gear and pin. It might be fine, and your issue is just the hub in front of it. You should be able to pull that off with a puller.

Not sure about Duramax, but BBC should have forward thrust load applied to the crank flange (say 100 lbs) with 1/2 thrust load (say 50 lbs) applied rearward to the main thrust cap prior to and during initial tightening of the main thrust cap fasteners.
