I should mentioned, the first time it happened, the van was sluggish, I turned it off and restarted it and it seemed better but not perfect. However, I didn't drive it last weekend and went to work Mon-Wed. with no problems.

We had our local dealer check it and they told us it was the 3rd gear pressure switch, which validated the research I did on the cause. The flashing D light means an intermitent failure in the transmission. When this happens the light will flash until the engine is shut off. If it was intermitent the flashing doesn't come back the failure was intermitent. However this shouldn't lock the gearshift out of the forward gears. That is probably a failure in the shift mechanism.

The 3rd and 4th gear transmission pressure switches seem to be a somewhat common wear item on the Pilot. See flashing shift indicator and DIY: Replacing Transmission Pressure Switches. Although I wasn't experiencing the flashing "D", I replaced these myself on my 2004 Pilot EX as a preventive measure a few months ago for ~$75 in parts.
